Vital Bridges Center on Chronic Care helps people throughout metropolitan Chicago impacted by HIV and AIDS to improve their health and build self-sufficiency. A program of Heartland Health Outreach, Vital Bridges addresses participants' comprehensive needs by providing food, nutrition counseling, housing, case management and prevention services.
