Description
Our mission is to help those in transition from rehab, hospitalization, shelter or half-way houses gain the necessary skills they need to find work, solid housing and other benefits to gain their self-esteem back. Our company name stands for "transition to moving forward." We offer 1-1 peer and group counseling with Domestic Violence, substance abuse problems, housing, HIV support and general case management. We also offer advocacy programs with Medicaid, SNAP, rental assistance programs and domestic violence.
