Description
San Francisco Adult Day Services Network is a collaborative of 15 adult day services centers in San Francisco and Marin counties. Together the Network’s member centers provide cost-effective, community-based services that helps nearly 1,500 seniors with chronic health problems or memory loss, and adults with disabilities, regain and maintain highest-capacity for self-care enabling them to remain in their homes and community and avoid premature institutionalization.
The Network also has another definition, as the convener organization providing leadership, coordinate tasks, develop initiatives and raise funds to assist the member centers to adapt and survive the challenges of a dynamic health care environment, increase access to care, eliminate health disparities and increase delivery of high quality, cost-effective services that have a measurable impact on health outcomes.
