Pueblo StepUp provides care management for low income children and seniors to help them access wellness, healthcare and well-being services.
Some of the direct services we provide:
Tai Chi for seniors
Chronic Disease Self Management Program
Monthly mobile wellness clinics
Enrolling eligible children into federal and state healthcare plans
Case managing children on Medicaid
