Description

We are a public charity, formed in 2003 to Conduct research to lessen the burdens of chronic life-limiting illness, and To develop and disseminate education directed at
improving patient and family experiences with chronic and life-limiting illnesses.
Our major project is Palliative Care-Heart Failure Education and Research Trials (PC-HEART) collaborative that works to provide education about symptom management and approaches to communication and improved care for persons with advanced heart failure.
