Description
Cancer and Chronic Disease program of whole person care for the patient, caregiver and family members. The program provides one-on-one specific counseling by master's trained "Pathfinders", volunteer meal and support help, networking for additional resources, patient advocacy and navigation through the medical system, end-of-life planning, non-denominational spiritual counseling, palliative and hospice care. The goal is to empower personal recovery for people experiencing cancer or a life-threatening illness.
