Medical Home Plus provides a leadership role in facilitating family, professional and community partnerships that improve the quality of life for children with special health care needs and /or disabilities - "connecting the dots" to create a more seamless system of service delivery. Its projects and programs reflect working partnerships with public and private agencies in multiple capacities.
