Description

Direct service programs for frail, homebound seniors in Fremont, Newark and Union City, promoting healthy lifestyle choices, supporting daily well-being and enabling independent living for as long as possible.

1. Meals on Wheels provides hot, nutritious home delivered meals, M –F (frozen meals for weekend), to homebound seniors who are unable to cook/shop for food.

2. Friendly Visitor Program matches homebound seniors with volunteers providing companionship through weekly visits, phone calls, and outings to help reduce isolation/depression.

3. (V.I.P. Rides) links volunteers with seniors or disabled individuals to assist/accompany on Dr. appointments, grocery shopping and errands using either local Paratransit or volunteer’s car for frail seniors requiring door thru door assistance.

4. Senior Falls Prevention provides in-home exercise, in-home falls risk assessment, minor home modifications and medication review to help prevent drug interactions.
