Health Hope Network (formerly Visiting Nurse Foundation) is a nonprofit based in Pittsburgh. Health Hope Network's primary program is Stroke Survivor Connection (SSC). The SSC is a network of 25 free weekly support groups for stroke survivors and their caregivers in Allegheny County and surrounding areas. Members meet for 3 hours weekly to participate in the 4 components of rehabilitation: physical exercise, cognitive development, emotional support, and social interaction. Following a stroke, many survivors find themselves isolated socially, depressed, and challenged by their physical and cognitive disabilities. Once the rehabilitation covered by their insurance has ended, they are left without a way to continue to improve their abilities. The SSC fills these gaps, offering stroke survivors emotional support, a social network of people with shared experiences, practical educational tools for daily living, and continued physical and cognitive rehabilitation.
