Green Tree Community Health Foundation is a non-profit dedicated to improving the health and well-being of our communities through partnering for change.
The Foundation, located in Germantown, is a nonprofit public charity striving for a positive impact on current and emerging health issues and risks, empowering residents in its 30-zip code, Eastern Montgomery County, and Northwest Philadelphia service area to access health services and to value, embrace and maintain their health. A community health advocate, the Foundation identifies areas of vulnerability in these communities and provides funding to organizations whose work addresses these needs.
The mission of the Green Tree Community Health Foundation is to improve the health status of community residents by initiating and supporting activities in response to identified needs in partnership with community resources.
