Description

Our organization is here to provide services to diabetics in St. Joseph County. We are especially sympathetic to those who are un and underinsured. We have a client advocate who assesses client's needs and helps them to enroll in programs for which they are eligible. While this process is taking place we give each client an allowance to use for supplies or medication through our company and partners. Our Diabetes Education Center provides one-on-one education on a sliding fee scale so that nobody is turned away from the education they deserve. We participate in community events and host community wide diabetes risk assessments throughout the county in order to screen our community for diabetes.
