Description

Community Visiting Nurse Association and its affiliates is a non-profit home healthcare agency that provides home health and support services (skilled nursing, therapy, home health aides and hospice) to residents in Middlesex and Somerset counties. Community VNA has been serving our communities since 1948, providing a wide range of health education and promotion, preventative care and acute care services to residents of all ages. Community Care Hospice provides hospice care to patients and their families who are in the final stages of their disease. Our interdisciplinary team of physicians, nurses, social workers, volunteers, clergy, therapists and home health aides provide palliative and support care.
