Community Health Professionals is a non profit home health care agency that provides comprehensive, progressive, community based, health care services to meet the needs of our communitiy. We believe in high quality, compassionate care that promotes independence dignity and safety. We also provide Hospice care. We strive to enhance the quality of life by focusing on pain and sympton control. Our Hospice care is administered under the supervision of the client's medical needs. We also minister to the emotional, psychological needs of both client and family.
