Our mission statement is to positively impact patients’ lives by providing resources to help them access quality healthcare and navigate the healthcare system in Northeast Ohio.
Carmella Rose Health Foundation was founded in 2004 out of the desire to help patients and families navigate the healthcare system. As a nonprofit 501(c) (3) organization based in Cleveland, Ohio, we help people access quality medical care during their healthcare journey.
Our Community Patient Navigator Program is unique because it is not connected to any one hospital system or federally-funded clinic. Our Community Patient Navigator, serves as the sole navigator for the City of Cleveland and Cuyahoga County, providing assistance to underserved and disadvantaged clients at 12 shelters and food pantries in the Northeast Ohio community. Presently we help around 80 patients per month, and that number continues to grow with the demand for our Community Patient Navigator program.
