Description
Alzheimer's Community Care is dedicated to providing community-based specialized, compassionate, quality care for persons with Alzheimer's disease and related disorders, and their caregivers. Our goals are to maintain quality of life after a diagnosis of Alzheimer's and to assist the caregiver in caring for the patient at home, thus delaying or averting nursing home placement. Alzheimer's Community Care places a safety net of services and resources around patients and caregivers every day. This safety net includes: Specialized Alzheimer's Day Care, Family Nurse Consultants, 24-Hour Crisis Line, Caregiver Support Groups, Case Management, Prescription Assistance, Community Education and Specialized Disaster Preparedness. All services are designed according to dementia-specific standards of care.
