ACTION Health will facilitate a community-based initiative to improve the health status and quality of life of the residents of Columbia, Montour, Northumberland, Snyder, and Union Counties of Pennsylvania. This will be accomplished by ongoing assessing of the health status of the communities, prioritizing the health needs of the communities, and facilitating a collaborative effort to improve quality of life.
